I am a darzi fellow who is working on a project with Kent Surrey and Sussex (KSS) AHSN Tackling Polypharmacy using system leadership across systems. For my project want to set up a medicines optimisation network across system and Professor Malby referred me to this platform. Would be looking at some ideas setting up a network which improves communication , relationships, share best practice and learning.
I am registered mental health nurse by background and have been working with people with Dementia and Long-term conditions in primary care.

Hi Sam, I'm a Patient Safety Manager working with the Oxford AHSN/PSC. One of my programmes looks to improve patient safety along the pathway of care of children with technologically dependent complex needs who are cared for at home. Their pathways start in the acute sector but once discharged into the community they have contact with multiple healthcare professionals - CCNs, dieticians, SALTs, school nurses, GPs etc. as well as frequently cycling back into and out of secondary care again. We are also looking to set up a multi-professional cross sector network/Community of Practice for all the reasons you give in your post and I would love to share ideas/thoughts/experience. I've scoped a few potential platforms but as yet have found nothing perfect.
